Why you received this notice
A federal income tax return, Form 1040 series, was filed under your Social Security number (SSN) or individual taxpayer identification number (ITIN).
We sent you a CP5071 series notice because we need you to verify your identity and the return:
- If you didn’t file a return: Verify with us. You may be the victim of identity theft.
- If you did file a return: Verify so we can continue processing your return.

If you can’t verify online
Call us at the phone number on your notice.
The number is for identity and tax return verification only. Other tax information, such as refund status, is not available.
- If you filed a return - Have ready when you call: a prior year tax return (if you filed one and have it available), the tax return for the tax year listed on the notice and supporting documents for each return (a W-2, 1099, Schedule C, F, etc.).
- If you didn’t file a return - Tell us when you call.
You can bring someone on the call to help you. You must grant authorization during the call for the IRS to disclose your confidential tax information to the third party. This oral authorization is limited to the conversation in which you provide the authorization. You must be present and participate in the call.

You’ll need to authorize power of attorney for a person who’s authorized to practice before the IRS. Either:
- Complete Form 2848, Power of Attorney and Declaration of Representative
- Have your representative send a power of attorney authorization request to your Individual Online Account. They can do this through Tax Pro Account.
When your representative calls, we encourage you to be available to help verify your identity.
If you’re acting as a fiduciary
If you're acting as fiduciary for an individual, a decedent’s estate or a trust, complete and send us Form 56, Notice Concerning Fiduciary Relationship.
How to protect your identity
To prevent someone from filing a return with your Social Security Number (SSN) or individual taxpayer identification number (ITIN), get an identity protection (IP) PIN.
If you verify your return, you 诲辞苍’迟 need to complete Form 14039, Identity Theft Affidavit, unless we tell you.
